To file a claim, please have these documents ready:

• Certified, colour copy of the Death Certificate*
• Return of Original Policy Documents or if unable to locate the beneficiary needs to complete the “Declaration of Lost Policy Form” .
• Proof of Death Physicians Statement
• Certified, Colour Copy of the Birth Certificate or Passport of the deceased.
• Certified, Colour Copy of the Marriage Certificate or Divorce Decree (if the deceased name is different than listed on file at time of death).
• Proof of Executor of Estate/Letters of Administration
• Claimant Statement
• Personal Verification Form – Beneficiaries.
• Certified, Colour Copy of Valid Photo ID (government issued – passport or driver’s license)
• Certified, Colour Copy of Marriage Certificate or Divorce Decree Certified (if the beneficiary’s name is different than listed on file at time of claim).
• Certified, Colour Copy of Valid Proof of Address (utility bill or bank/credit card statement, cannot be a mobile/cell phone bill)
• FATCA (Foreign Accounts Tax Compliance Act)/CRS (Common Reporting Standards) Form
